One form covers every programme. You do not need to pick the exact programme — screening decides.
Initial screening within two weeks
A call or short appointment to understand the case and set priority under published criteria that put the greatest need first.
Specialist assessment
A multidisciplinary assessment setting the type, intensity and expected duration of the intervention.
Individual plan and service start
A written plan, a copy of which goes to the family, reviewed every three months against measurable indicators.
The form
Request details
A beneficiary’s health data is special-category data in law. Only the screening and assessment team can see it; it is never published or shared outside the association.
No medical reports are needed now. They are requested later at the specialist assessment, if required.
